Mobility Match takes a patient centered approach to provide customizable services and increase the likelihood of sustainable appointment adherence. By working directly with the patient to identify their needs, as they relate to transportation inequities, we are able to connect patients to the best transportation option and ensure they are able to get to and from their health destination.

There are many things that transportation helps put into place so patients can get to their health care appointments. We will identify patient needs and provide the appropriate education, resources, and/or referrals to ensure patients have the necessary tools to get to their appointments.
The Mobility Match multimodal transportation platform will help coordinate services and connect patients to the most appropriate transportation option, ensuring they are able to make it to and from their health destination.
Mobility Match will follow-up within 24 hours of each scheduled trip, and monthly, to evaluate our services and identify any additional resources needed to ensure patients are able to maintain a continuum of care.
